2007/08 St. Isidore’s Faith Formation
Instructions for Registering Online
Please follow these steps to register:
- Print out one copy of the Registration Form & ER Consent Form for each child.
- Completely fill out all forms by hand and sign where necessary.
- Consult the Programs Fees form.
- You can pay by either check (made out to St. Isidore’s) or by printing out and completing the Credit Card Authorization form.
- Please consider volunteering, there are many options listed on the Volunteer form.
- Mail or drop off completed registration packet to the Faith Formation Office.
- A completed packet contains:
- Registration Form (one per child)
- ER Consent Form – 2 pages (one per child)
- Payment
- Volunteer Form (optional)
